Evaluasi interaksi obat potensial pada terapi pasien rawat inap di Rumah Sakit UMM

Peningkatan kompleksitas terapi pada pasien rawat inap meningkatkan risiko terjadinya interaksi obat yang dapat memengaruhi efektivitas maupun keamanan pengobatan. Penelitian ini bertujuan mengidentifikasi dan mengevaluasi potensi interaksi obat berdasarkan jumlah kejadian, mekanisme, tingkat keparahan, dan rekomendasi penyelesaian pada pasien rawat inap di Rumah Sakit Universitas Muhammadiyah Malang. Penelitian dilakukan secara observasional retrospektif menggunakan data rekam medis pasien bulan Desember 2021 dengan total 32 sampel pasien yang menerima minimal lima obat. Hasil analisis menunjukkan bahwa (65,6%) pasien mengalami lebih dari satu interaksi obat potensial. Mekanisme interaksi terbanyak adalah farmakodinamik (62,2%), diikuti farmakokinetik (30,6%). Berdasarkan tingkat keparahan, interaksi kategori moderat mendominasi (65,3%), sedangkan mayor sebesar (21,4%) dan minor (13,3%). Rekomendasi penyelesaian terbanyak adalah pemantauan ketat (monitoring closely) sebesar 58,2%, disusul dengan penggantian obat dan penyesuaian dosis. Hasil penelitian ini menegaskan pentingnya peran farmasis klinik dalam mengidentifikasi dan memantau interaksi obat untuk meningkatkan keselamatan dan efektivitas terapi pasien di rumah sakit.
